Grant Application Procedures
The foundation does not have an application
form, but we must have the following information to review your
request for funds:
- A summary page that briefly describes the organization,
summarizes the request, gives the total budget of the organization,
the project budget (if different), and the amount of your grant
request.
- Description of the organization, including
its history, membership structure, current issues being addressed
and planned action strategies.
- Description of leadership development and accountability,
including how leaders and staff members are trained, how issues
are chosen and decisions made, and how the organization relates
to the community.
- The board of directors, including a list of
officers, and their work and community affiliation.
- Staff list, including title or position.
- Job description for community organizer, or
comparable position, if applicable.
- Financial information (please include
all items listed below):
- The organization's current year budget of anticipated
income and expenses, including a list of anticipated/committed
income from foundations or corporations.
- The budget of anticipated income and expenses
of the project for which you are requesting support, if different.
- A financial report: year-to-date actual income
and expenses for the current fiscal year, a balance sheet,
and a listing of sources of revenue.
- A copy of the most recent audit, or unaudited
end-of-year financial statement, including balance sheet.
- Internal Revenue Service tax-exempt ruling letter.
Please limit your proposal narrative
(excluding financial information) to no more than ten pages, using
at least a 12-point font. It is helpful if the proposal is not bound/stapled
or printed on both sides of the page.
Deadline for applications: The board of directors of the Wieboldt Foundation meets six times a year. For 2008 the following deadlines for proposal submission will apply: November 30, 2007 for review on January 31; February 1 for review on April 10; April 4 for review on June 5; June 6 for review on August 7; August 1 for review on October 2; October 3 for review on December 4; and November 28 for review in January 2009.
We prefer to receive proposals via e-mail if they are in a Word
document. You may send the financial information required via e-mail
if it is in an Excel document, or you may also send your proposal
and attachments via regular mail to Wieboldt Foundation, 53 W Jackson
Blvd., #838, Chicago IL 60604. We do not accept faxed proposals.
